Description
The Denzel Group has been chosen to work with an Industry Leader on their Sr Epic Healthy Planet opening.
This full time FTE position is hybrid. Onsite schedule is flexible.
In this role, you will be focused on meeting all the technical needs for the network's clinical pathways, care management functions, value-based care programs, and community health needs. Many of the projects this person would help complete will have a direct impact on network goals, including lowering readmission rates and improving transitions of care.
Requirements
5+ years direct Epic Healthy Planet or Ambulatory build experience
Extensive experience working with technology in the Healthy Planet and population health areas
Epic Healthy Planet Certification (preferred)
Clinical Outpatient Care experience (Plus)
Epic Ambulatory certification (Plus)
Epic Inpatient certification (Plus)
Epic Healthy Planet / Epic Ambulatory / Epic Inpatient
